Through this program, we are able to serve elderly and physically disabled residents in the District of Columbia receiving EPD Waiver services. Our licensed case managers support our clients in accessing person-centered services in their home and community. We seek to coordinate, navigate & advocate to fulfill the needs and preferences of each client served.
